Complete Your Patient Profile

Before you can schedule your first visit, you'll need to complete your patient profile. This ensures our providers have the information they need to deliver excellent care.

Required Information

Personal Details

  1. Navigate to My Profile from the side menu
  2. Fill in the following information:
    • Full Name (First, Middle, Last)
    • Date of Birth (MM/DD/YYYY format)
    • Gender (Male, Female, or Prefer not to specify)
    • Phone Number (Mobile number required)
    • Email Address (Already set from registration)

Profile Photo

A profile photo helps our providers identify you during your visit:

  1. Tap Add Photo in your profile
  2. Choose to either:
    • Take a new photo with your camera
    • Select an existing photo from your device
  3. Crop and adjust as needed
  4. Tap Save

Note: Your photo is only shared with assigned providers for identification and safety purposes.

Government ID Verification

For security and compliance, we require identity verification:

  1. Navigate to Identity Verification in your profile
  2. Upload a photo of your:
    • Driver's License, or
    • State ID Card, or
    • Passport
  3. Ensure the photo is clear and all information is readable
  4. Tap Submit for Verification

Your ID will be reviewed within 24 hours.

Address Information

Add Your Primary Address

  1. Tap My Addresses in your profile
  2. Tap Add New Address
  3. Enter your address:
    • Street address
    • Apartment/Suite number (if applicable)
    • City
    • State
    • ZIP code
  4. Add any Special Instructions for providers (e.g., "Use side entrance", "Gate code: 1234")
  5. Set this as your Default Address
  6. Tap Save

Tip: You can save multiple addresses (home, work, family member's home) for convenience.

How We Use Your Address

  • We use Azure Maps to verify your address and calculate provider travel times
  • Your exact address is only shared with your assigned provider
  • Geographic coordinates help us match you with the closest available providers

Emergency Contact

An emergency contact is required for all in-home healthcare services:

  1. Navigate to Emergency Contacts
  2. Tap Add Emergency Contact
  3. Enter:
    • First and Last Name
    • Relationship (Father, Mother, Spouse, Friend, etc.)
    • Mobile Phone Number
  4. Tap Save

Important: This contact may be called in case of medical emergency during your visit.

Legal Compliance

Before scheduling visits, you must review and accept:

  1. Privacy Policy - HIPAA compliance and data protection
  2. Terms & Conditions - Service agreement and platform usage

To review and accept:

  1. Navigate to Legal Documents in your profile
  2. Tap each document to read
  3. Scroll to the bottom
  4. Tap I Accept for each document

Phone Verification

Verify your mobile phone number:

  1. You'll receive a verification code via SMS
  2. Enter the code in the app
  3. Tap Verify

This enables important appointment notifications and provider communication.

Profile Completion Checklist

Before scheduling your first visit, make sure you've completed:

  • ✅ Personal details (name, DOB, gender, phone)
  • ✅ Profile photo
  • ✅ Government ID verification (submitted)
  • ✅ At least one address
  • ✅ Emergency contact
  • ✅ Legal documents accepted
  • ✅ Phone number verified

Once your profile is complete, you can:

  • Add your insurance information (recommended)
  • Add family members for coordinated care
  • Schedule your first visit
